Just looking at Washington State and British Columbia will give you a long list of current ferries in NW America. I believe they are all car ferries, not passenger only.
In Washington they are run by the same state Dept. that runs the roads, they are essentially a direct part of the road system.
Looks like 11 routes in WA -

British Columbia seems to be privatized, and too many for me to count...:)

This is a picture of mine from 2008 - Friday Harbor on San Juan Island - and the only way in if you don't fly -

But this is how I got around, much quicker at 150 mph....:)
On approach to Powell River in British Columbia, on the mainland but still two ferry rides and 5 hours driving from from Vancouver.

Well, since non-one has jumped in with other ferries around the world - I'll add in Southern California -
There is the Catalina Ferry - I don't think private cars are allowed on the island - this ferry is people only -
http://www.catalinainfo.com/
..and even a little one across Newport Beach Harbour - we took it once just for fun - three cars at a time for a 800 ft crossing...:)

Come on Yo folks, add some more operating ferries from around the world....:PDT_Aliboronz_24:

I thought I would add another one to this old thread. When we were in New Orleans 2 weeks ago, I took this pic. of the ferry across the Mississippi at the down town area.
It seemed to be an unusual configuration, it is tied up with the starboard side to the dock - cars entered at the stern and moved down the port side to the car exit on the bow end of the starboard side.

It's a short ride across the river there, and is largely obsolete due to the freeway bridge just 1/2 mile up river. It was free to pedestrians and only a dollar for cars in one of the directions.
It's operated by the state and has been running at this location for a long time - since 1827 -

When we left, Air Traffic Control wasn't too busy, so I requested, and got, a photo pass in the controlled area up the river at 1500 ft. The ferry is at the right side of the river front building with the "disk" on top - the Harrah's Casino. It shows how the ferry is in the middle of the business district.
Just to the right of the tall buildings is the original part of New Orleans - the historic district and the French Quarter.

We have a couple of ferries "On the swan river in Perth Western Australia" they go to a little Island off Fremantle WA, it's called ROTTNEST it's our Holday island something like the "isle of man" thee only diffence is ours have No vehicles except Emergency and service vehicles, you either get around by bicycles, walking, or taking one of the tour buses. or there is a puffin billy train but that only goes to the old WW2 Gun Batteries which are appox 3 miles under ground & built around 1938/39, they also have a ferry that was built in 1910 that has been refurbished many many times but has always kept to the orginal design,

I'll add another one that made the news to-day, but only just makes this list since it under the budget axe.
Rocky Hill, Connecticut.
22131
It's a bit unusual since it's an unpowered barge sized for four small cars with a small boat (tug?) attached to it's side.
It made the news because it's the oldest continously running ferry in the US - started in 1665 with pole boats.
http://connecticut.cbslocal.com/2011/07/15/governor-dannel-malloy-puts-nations-oldest-ferry-on-chopping-block-in-connecticut-budget-proposal/
I'm sort of guessing the the highway bridge crossings 6 miles North and 4 miles South didn't exist in 1655 though...:)
